CERTIFICATION OF PHARMACY INTERN HOURS FROM ILLINOIS TO
ANOTHER STATE:

  • Must hold an active Illinois Pharmacy Technician license at the time hours were completed.
  • Must furnish a letter from your supervising pharmacist in Illinois on letterhead stationary stating the starting date and the ending date of employment and the total number of hours worked. The supervising pharmacist then must sign and date this statement. Check stubs are not acceptable. If you just graduated from College, furnish a letter from a Dean or Professor on letterhead stationery stating the total number of hours of practical training completed. Your Social Security Number must also be included in the letter. Also indicate in your letter which State you would like the letter to be sent to.

Once the information requested is received, a letter will be sent directly to the State. If you would like a copy of the letter, please indicate so in the letter and provide a mailing address.
